PROGRAMS OFFERED:
Identify needs, link, and coordinate services for consumer
Develop individual service plans, monitor consumer’s progress, update service plan as needed
Assist consumer to identify and prioritize life goals and necessities
Advocate for the consumer as needed
ADMISSION REQUIREMENTS:
Resident of Mecklenburg County
18 years old and older
Meets State guidelines for this level of service
PROGRAMS OFFERED:
Substance abuse evaluation, referrals for outpatient and inpatient substance abuse treatment relapse prevention, referrals for employment, medical transportation, and a demonstrated inability to maintain abstinence outside of an institution
